Acceptance of Clients. Affordable Home Care Services, Inc. has provided several satisfied clients with our services since May 2006. We accept clients based on their desire or need for household or personal support and/or companionship services. For a list of services that we can and cannot provide, please refer to the information provided in the document entitled "Introduction: Home Services Agency," which is included in this packet. Affordable Home Care Services, Inc. will ensure that no person shall be refused services based on age, race, color, sex, marital status, sexual orientation, or national origin.

Initial assessment: The purpose of the initial assessment is to gather information about the client in order to help establish the best possible plan of care. During the initial assessment, we will ask for personal information (name, address, phone number, etc.), contact information (emergency contact, primary doctor information, etc.), and a brief medical history. We will also perform a needs assessment to determine what type of services needed to be offered, or if more advanced services, which would require a home health agency, are needed. The initial assessment will be performed by phone or on-site, if necessary. The initial assessment will be performed at no charge to the client.

Service plan: After reviewing the initial assessment with the client and his or her appropriate family members or representative, we will establish a service plan that will outline the services to be provided. The plan will address the level, type, and/or scope of services the client is receiving; identification of any functional limitations of the client and the relevance of the limitation to the services to be provided; information received from the client, in consultation with the client and his or her appropriate family members or representative, during the initial assessment (such as medications being taken, treatments received, client's physical activity, diet, mental status, etc.), hours of care to be provided; and the cost of care to be provided. The service plan shall be reviewed and revised as necessary.

Discharge of Clients. The client or Affordable Home Care Services, Inc. will have the right to terminate services at any time. For more information about the termination of services, refer to the client service agreement, paragraph 15. When services are terminated by the agency, the client is to be notified at least seven working days in advance of the date of termination, with a stated reason for the termination. The seven (7) day notice is not applicable in cases in which the home services worker's safety is at risk. In such cases, the agency will notify the client of termination of services immediately and the reason for termination.

Home Services Workers. It is the responsibility of Affordable Home Care Services, Inc. to ensure that we are providing the best possible home services workers. Affordable Home Care Services, Inc. is in substantial compliance with the Health Care Worker Background Act [225 ILCS 46] and the Health Care Worker Background Check Code [77 Ill. Admin. Code 955]. Before we hire a home services worker, we ensure that the worker does not have a disqualifying background check under the requirements of the Health Care Worker Background Check Act without a waiver; has a copy of a social security card; and has proof a citizenship in compliance with federal requirements for employment. In addition, all direct-care staff that is non-licensed has been checked with the health care worker registry. Furthermore, all home services workers are provided eight hours of training prior to his or her first assignment.

Payment of services. Affordable Home Care Services, Inc. will bill its clients every two weeks. It is expected that payment is received in a timely manner. Payment for services may be arranged as follows:
- Direct Payment Plan: Payments will be automatically deducted from the check or savings account specified on the contract (refer to the Service Agreement paragraph # 2- Payment for services) through Direct Payment Plan. Proof of payment will appear on your statement that you receive from AHCS. If amount of your payment changes, we will credit the balance to your account during the next billing period.
- Payment by check: In addition, the client has the option of paying directly by check. It is expected that all payments be rendered upon receipt of the invoice. Payments received 10 days past the due date provided in the invoice will be assessed a late fee of $35.
The client application process will consist of three phases: initial assessment, development of service plan, and finalization of client service agreement. The different phases are described below:
Finalization of Client Service Agreement: After the client and his or her appropriate family members or representative aggress to the service plan; a client service agreement will be completed. The client service agreement is a contact between Affordable Home Care Services, Inc. and the client. This contract will remain in effect until the termination of the services by either the client or Affordable Home Care Services, Inc. The client service agreement must be completed before the provision of services will begin.
Depending on the insurance that you carry, it is possible that your health insurance may pay for some or all of the services you receive. It is your responsibility to contact your insurance company to determine if they will provide payment for these services. In this case, you will be responsible for pay Affordable Home Care Services, Inc. directly, and your insurance would have to reimburse you for the payment.
