Affordable Home Care Services, Inc. is dedicated to providing safe, quality, and cost effective home health care services to all clients. Our goal is to meet the client's medical needs in the nurturing environment of his/her home, minimize the impact of the client's condition on the family, and respect the role of the family in the care of the client.

HOME HEALTH CARE PHILOSOPHY

POLICY:

The provision of home health services is guided by the philosophy of AHCS.

PURPOSE:

  1. To adopt the appropriate philosophical approach in providing quality home health care services
  2. To ensure quality home health care to patients.
  3. To provide continuity of home health care for patients.

PROCEDURE:

The adoption of the appropriate philosophical approach in providing quality home health care services based on the following basic assumptions.

  1. The maintenance and realization of the full potential of human life is a supreme value.
  2. Humans possess a unique hierarchy of needs, as defined by Maslow:
    • Physiological needs
    • Safety needs
    • Belonging and love needs
    • Self-Esteem needs
    • Self-Actualization
  3. Humans search for meaning in personal life experiences.
  4. Humans make choices and decisions based on their individual beliefs and values.
  5. Humans exert personal control over their lives in making such choices and decisions.
  6. The adoption of the appropriate philosophical approach in providing quality home health care services based on the following basic assumptions.

The philosophy of the agency is based on the above assumptions, stipulates that each home health care patient:

  1. Desire home health care services that promote the patient's value of life by:
    • Minimizing negative patient illness and disability outcomes.
    • To ensure quality home health care to patients.
    • To provide continuity of home health care for patients.
    • Maximizing potential patient level-of-independence outcomes.
    • Restoring, maintaining, and promoting patient health.
  2. Possesses unique physiological, safety, psychological, self-esteem, and self-actualization needs that require consideration in the patient's plan of home health care.
  3. Searches for meaning in the personal life experiences surrounding the patient's illness or disability.
  4. Make choices and decisions about the provision of home health care services relative to the patient's self defined unique needs and according to the patient's personal beliefs and values.
  5. Exert personal control over the patient's personal life in collaborating with home health care staff members regarding the provision of services.

In support of the Agency's philosophy, employees are dedicated to the provision of comprehensive quality care which is patient-centered and patient family-centered, provided in the patient's place of residence.Care focuses on the patient's unique physiological, safety, psychological, self-esteem, and self-actualization needs. The agency is committed to the provision of home health services that assist the patient to search for meaning in the personal life experiences surrounding the patient illness or disability. Staff helps patient make choices and decisions about the provision of home health care services based on the patient's self-defined unique needs. Patients and families are encouraged to collaborate activity with agency staff and other health care providers regarding the provision of appropriate services. Education to the patient, the patient's family, and the community of health related issues is also part of the Agency philosophy.
